OCRA Review Submission Instructions

To help us process your request efficiently, please complete the OCRA Review Form using the process outlined below.

1. Complete the form. Click the link below to access the OCRA Review Form. It should take approximately 5 minutes to complete.

2. Save this link and your submission. You will need access to your original submission in order to upload additional documents later, so please save this link along with a copy of your submission.

3. Save your confirmation email. After submitting the form, you will receive an email confirmation. Convert this email to a PDF and upload it to your eIRB application as proof of your OCRA submission.

4. Use Adobe Sign for signatures. Please use Adobe Sign for any documents requiring a signature. Do not use DocuSign, as it will lock the form and prevent additional signatures.

5. IRB approval is required before UH review. UH will not begin reviewing your study until an IRB approval letter has been received. You will receive a reminder email prompting you to upload your IRB approval letter to the form once it is available.

6. Review process. Once we receive your complete documents, including IRB approval, we will review them and contact you if any additional information is needed.
